Dear Patient,

Our vision

Our vision is of a borough where health and social care provision is equal and accessible to all; where services are high quality and meet the needs of all Hackney communities and where residents are at the heart of the design, delivery and improvement of health and social care services.

Our mission

Our mission is to improve health and social care provision and outcomes for people in Hackney by working to ensure that treatment and care is provided with respect and dignity, valuing diversity, encouraging participation and working together. We do this by being the independent champion for residents and people who use services ensuring the voices of people across the borough are heard in order to influence decision makers.

CITY & HACKNEY CARERS CENTRE

City and Hackney Carers Centre supports the unmet practical, emotional and social needs of carers.

An unpaid carer is someone of any age who, without payment, provides care and support to a partner, friend, relative or neighbour who could not manage without their help. This may be due to a physical or learning disability, a mental health condition, long term illness, frailty, addiction or substance misuse.

SOCIAL PRESCRIBING SERVICES

There are many things that impact our health and wellbeing, including feeling isolated or struggling with practical challenges. We have Social Prescribers in our practice who can speak with adults registered with the practice. They will ask you about what is happening in your life, what you enjoy, and what is getting in the way. The Social Prescriber might suggest services and activities that might help you or simply work with you to build your skills and confidence. They can tell you about community based services, e.g. lunch clubs, gardening groups, benefits advice, exercise groups.

Please ask your GP to refer you to social prescribing.

FGM (Female Genital Mutilation):

It is a mandatory duty for a regulated healthcare professional to report any concerns they have about a female under 18 years and record when FGM is disclosed or identified as part of NHS healthcare. FGM is usually carried out on young girls between infancy and the age of 15, most commonly before puberty starts. It's illegal in the UK and is child abuse. It's very painful and can seriously harm the health of women and girls. It can also cause long-term problems with sex, childbirth and mental health.
